Instructions for using this order form:

Please follow all steps to ensure order accuracy.

1 - Enter appropriate company info (name, address, PO#, etc...) in the fields below. Use the tab key to move from one field to the next.

2 - Select the product from the pop-up list, then manually enter quantity and price. (This website does not contain any pricing information, so you will need to enter the prices yourself . Please consult your SpringStar price list for exact prices and case quantities.)

3 - Click on the "equal sign" box for each row to see the total for that line.

4 - Once you have completed and reviewed your order press "Calculate Total" to see your total. Please note that shipping charges are not included in the total on this order form. Shipping charges will be calculated once we have processed your order.

5 - Once you have completed you order, print this page for your records and click the "Submit You Order to SpringStar" button.
